Generally speaking, the current NH Rules for the Education of Children with Disabilites states that goals are required to have objectives, the steps to achieve them. As needed is a very vague statement - as needed determined by who? Your child or the councelor? The IEP can be revisited to list a specific number of hours (units) your child will receive this service. As for your final question, if the team has determined that a certain approach or service is required and listed in the IEP, it must be delivered.
